WAIVER AND RELEASE OF LIABILITY
In consideration of being allowed to use the soccer fields, facilities and equipment at the Portage Community Soccer Complex, Portage Soccer Club Practice Fields, Portage Public Schools, and SoccerZone, Southwest Michigan Pickle Ball, property located at 52755 Johnson Rd, Three Rivers, MI 49093, Corey Talcott Rentals, LLC,
the undersigned:
1. Agrees that prior to using the soccer fields the undersigned will inspect the facilities and equipment to be used, and if the undersigned believes anything is unsafe, the undersigned will refuse to use the soccer fields; and
2. Acknowledges and fully understands the undersigned will be engaging in activities which involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from my actions, inactions, negligence of others, the rules of play, or the condition of the premises or of any equipment. Further, there may be other risks not known to me or reasonably foreseeable at this time; and
3. Assume all of the foregoing risks and accept personal responsibility for any damages following such injury, permanent disability or death; and
4. RELEASE, WAIVE, DISCHARGE, AND COVENANT NOT TO SUE the Portage Soccer Club, Pharmacia & Upjohn, L.L.C., City of Portage, Portage Public Schools, SoccerZone, Southwest Michigan Pickle Ball, property located at 52755 Johnson Rd, Three Rivers, MI 49093, Corey Talcott Rentals, LLC, and their respective employees, officers, and representatives (RELEASEES), from any liability to the undersigned, my heirs and next of kin for any and all claims, demands, losses or damages (CLAIMS) on account of loss injury including death or damages to property, caused or alleged to be caused in whole or in part by the RELEASEES and will defend, indemnify and hold harmless RELEASEES from and against and all such CLAIMS.
The undersigned have read the RELEASE and understand the effect of the terms and conditions of the RELEASE and also understand this RELEASE shall be in effect for any and all activities related to the use of the soccer fields, facilities and equipment."

We, the participants and the guardian, Release and forever discharge the proprietor and land owners (Corey Talcott Rentals, LLC) from any and all liability, claims, and demands of whatever kind of nature, either in equity or in law. That includes but is not limited to whichever may arise or hereafter arise from the activities as a participant, bystander, or have entered the property located at 52755 Johnson Rd, Three Rivers, MI 49093.
We understand and acknowledge that this release discharges any member of the Talcott Family as well as any subsidiaries or businesses associated with the property or owner, from any liability or claim that may arise with respect to bodily injury, personal injury, illness, death, or property damage that may result from participation or occupancy. This includes but is not limited to World Champion Soccer Academy, Lindsay Tarpley Snow, BJ Snow, Southwest Michigan Pickle Ball, etc.
I, the participant and the guardian also affirm and understand that I am responsible for the medical bills if any injury occurs. I also understand that this residence and/or its' owners do not assume any responsibility for, or obligation to provide any financial or other assistance, including but not limited to medical, health or disability benefits or insurance of any nature in the event an injury, illness, death, etc, occurs on this property. We expressly waive any such claim for compensation or liability on behalf of the aforementioned parties.
As such, any and all property located at the above address to include, but not limited to sports barn, parking lot, driveway, apartment, commercial equipment (heavy duty, light duty or similar), etc imposes an inherent risk being on or near any of these. I am aware of the dangers associated with being on this property and that from here on out, it is play-at-your-own-risk. Any and all sports, activities, or lounging around inside the barn, house or accessory buildings can result in serious injury or death.
During Covid-19, it is furthermore understood that it is my responsibility as an individual and/or guardian to protect yourself and dependents. It is also necessary to note that during winter months the sidewalks and driveways are not regularly shoveled or salted, and that any damage that may arise to my property, vehicle, or body, is my own responsibility year round.
By signing this form I am acknowledging that I have fully read and understand the terms, and that this agreement is under enforcement now and every time I enter said property for the duration of my existence. Upon signing, I am also acknowledging this goes for my immediate family. If you prefer to not sign or abide by the rules, we respectfully ask that you do not enter the property, and by default would be considered trespassing if entered without signing.
I grant permission to World Champion Soccer Academy, hereinafter known as the "Media" to use my image (photographs and/or video) for use in Media publications including: pictures, videos, promotional items, etc.
I hereby waive any right to inspect or approve the finished photographs or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.
I am the parent or legal guardian of the below named child. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release. I understand that I am free to address any specific questions regarding this release by submitting those questions in writing prior to signing, and I agree that my failure to do so will be interpreted as a free and knowledgeable acceptance of the terms of this release.

WHAT IS A CONCUSSION?
A concussion is a type of traumatic brain injury which changes the way the brain normally works. A concussion is caused by a fall, bump, blow, or jolt to the head or body which causes the head and brain to move quickly back and forth. A concussion can be caused by a shaking, spinning or a sudden stopping and starting of the head. Even a “ding,” “getting your bell rung,” or what seems to be a mild bump or blow to the head can be serious. A concussion can happen even if you haven’t been knocked out.
You can’t see a concussion. Signs and symptoms of concussions can show up right after the injury or may not appear or be noticed until days or weeks after the injury. If the player reports any symptoms of a concussion, or if you notice symptoms yourself, seek medical atten- tion right away. A player who may have had a concussion should not return to play on the day of the injury and until a health care profes- sional says they are okay to return to play.

• Confusion “Feeling Down” • Not “Feeling Right” • Slow Reaction Time • Sleep Problems
IF YOU SUSPECT A CONCUSSION:
1. SEEK MEDICAL ATTENTION RIGHT AWAY – A health care professional will be able to decide how serious the concussion is and when it is safe for the student to return to regular activities, including sports. Don’t hide it, report it. Ignoring symptoms and trying to “tough it out” often makes it worse.
2. KEEP YOUR STUDENT OUT OF PLAY – Concussions take time to heal. Don’t let the student return to play the day of injury and until a heath care professional says it’s okay. A player who returns to play too soon, while the brain is still healing, risks a greater chance of having a second concussion. Young children and teens are more likely to get a concussion and take longer to recover than adults. Repeat or second concussions increase the time it takes to recover and can be very serious. They can cause permanent brain damage, affecting the student for a lifetime. They can be fatal. It is better to miss one game than the whole season.
3. TELL THE SCHOOL ABOUT ANY PREVIOUS CONCUSSION – Schools should know if a player had a previous concussion. The player’s club/team may not know about a concussion received in another sport or activity unless you notify them.
SIGNS OBSERVED BY PARENTS:
• Can’t recall events prior to or after a hit • or fall • • Is unsure of game, score, or opponent •
• Moves clumsily
• Appears dazed or stunned
• Is confused about assignment or position
• Forgets an instruction
CONCUSSION DANGER SIGNS:
• Answers questions slowly
• Loses consciousness (even briefly)
• Shows mood, behavior, or personality changes
In rare cases, a dangerous blood clot may form on the brain in a person with a concussion and crowd the brain against the skull. A player should receive immediate medical attention if after a bump, blow, or jolt to the head or body s/he exhibits any of the following danger signs:
• One pupil larger than the other
• Is drowsy or cannot be awakened
• A headache which gets worse
• Weakness, numbness, or decreased coordination
• Repeated vomiting or nausea
• Slurred speech
• Convulsions or seizures
• Cannot recognize people/places
• Becomes increasingly confused, restless or agitated
• Has unusual behavior
• Loses consciousness (even a brief loss of consciousness should be taken seriously.)
HOW TO RESPOND TO A REPORT OF A CONCUSSION:
If a player reports one or more symptoms of a concussion after a bump, blow, or jolt to the head or body, s/he should be kept out of athletic play the day of the injury. The student should only return to play with permission from a health care professional experienced in evaluating for concussion. During recovery, rest is key. Exercising or activities which involve a lot of concentration (such as studying, working on the computer, or playing video games) may cause concussion symptoms to reappear or get worse. Students who return to school after a con- cussion may need to spend fewer hours at school, take rests breaks, be given extra help and time, spend less time reading, writing or on a computer. After a concussion, returning to sports and school is a gradual process which should be monitored by a health care profession- al.
Remember: Concussion affects people differently. While most players with a concussion recover quickly and fully some will have symptoms lasting for days, or even weeks. A more serious concussion can last for months or longer.
